  1. Purchase an FDA approved instant test kit and leave it sealed (Click this link for a list of FDA approved tests)
  2. Call us at 916-487-3152 x9 Monday through Friday from 8am – 5pm to schedule and pay to reserve your appointment.
  3. On the day of your appointment bring a government issued form of ID along with your sealed test kit. Park outside the Collection Plus office and call us to let us know you’ve arrived. Stay in your car and we will come to you.

The cost to you is only $20.00.

WE WILL PROVIDE YOU WITH A SIGNED DOCUMENT VERIFYING:

  1. Your identity
  2. Use of an FDA-approved test kit
  3. The test specimens were supervised throughout the entire process
  4. No other specimens were present in the collection area
